Forms

Module “Forms” allows user administrate forms and filled forms. Module includes three parts: “Forms”, “Field groups” and “Filled forms”. Filled forms are the results from of the forms filled by the visitors on the website.

 Field groups

“Field group” is a selection of values. For instance you want visitor to choose if he/she is a man or woman – in this case you have to create field group “Sex” and insert values “Man” and “Woman”. By adding field group user can:

  • insert the name of the fields group;
  • insert values. Values should be separated by semicolons;

Action form under the list allows to delete field groups.

Forms

Adding a form is different from any other entry adding in CMS. The page consists of two parts: “Form options” allows to add form basic data and “Form structure” allows to add unlimited number of fields, texts and menus. By adding form options user can:

  • choose a page, where the form will be added to. One page can include only one form;
  • insert the name of the form;
  • insert e-mails, to which the filled forms will be sent. E-mails should be separated by semicolons;
  • choose if the form is hidden;
  • choose if the form should include control code (captcha).

When adding form structure user has to choose one of four types and click “Add block” button – so one part of the form is created, where user can choose options.

Selection “Field” adds field to the form. By adding field user can:

  • insert the name of the field;
  • choose field style: either this is one-row or multi-row text field;
  • choose if the field is required to be filled on the website.

Selection “Group” adds a menu with choices to the form. By adding group user can:

  • insert the name of the field;
  • choose field group;
  • choose field style: either this is drop-down menu (Select) – allows visitor to choose one answer from several, radio button (Radio) –  allows visitor to choose one answer from several, check box (Check) – allows visitor to choose several answers;
  • choose if the field is required to be filled on the website.

Selection “Text” adds text. By adding text user can:

  • insert text;
  • choose the size of the font: normal, bold or big (Header).

Selection “File” adds browsing field to the firm. By adding field user can:

  • insert name of the field;
  • choose if the field is required to be filled on the website.

Each block has buttons for moving up/down and deleting.

In the list of forms it’s possible to filter forms by the pages.

Action form under the list allows to delete and copy forms.

Filled forms

This page displays the list of forms filled by visitors. User can see the results of each filled form: the time and date of filling the form and the selection of answers.

In the list of filled forms it’s possible to filter them by the forms.
